2. Structure

**16 questions**, **4 capabilities × 4 questions**
Order in `CAPS[]`: Decision-making (`dm`), Execution (`ex`), Adaptability (`ad`), Resilience (`rs`)
Scale labels (buttons show **1–5** visually): Never, Rarely, Sometimes, Often, Almost always
**Score values stored:** **0, 1, 2, 3, 4** (button index − 1)

3. Exact questions (from `CAPS` in the HTML)

Decision-making (`dm`) — Making clear calls with the facts you have.

1. I can make a clear call without rereading everything twice.

2. When I have enough facts, I decide instead of waiting for more.

3. I take a breath before I make a big call under pressure.

4. I feel good about my decisions the next day.

Execution (`ex`) — Turning decisions into done.

1. I start my day knowing my top priority.

2. I can focus on one task without getting pulled away.

3. I finish what I say I will do, even in busy weeks.

4. I turn decisions into clear next steps for my team.

Adaptability (`ad`) — Shifting well when the plan changes.

1. When plans change, I adjust without losing my footing.

2. I can hear hard feedback without getting defensive.

3. I can switch between tasks and still think clearly.

4. I see change as something I can work with.

Resilience (`rs`) — Bouncing back and staying calm under pressure.

1. I bounce back from a hard day by the next morning.

2. I stay calm when a meeting gets tense.

3. I have a simple way to reset when stress builds.

4. I wake up feeling rested most days.

capability_sum   = sum of 4 answers for that capability   // 0–16

capability_score = Math.round(capability_sum / 16 * 100)  // 0–100

overall          = Math.round(average of 4 capability scores)

Copy for each band comes from CAPS[i].band (and cardMid for mid Decision-making on cards).

Focus capability (lowest score)

Focus = capability with the **lowest** score.
**Ties** broken in this order: **Resilience (3) → Decision-making (0) → Execution (1) → Adaptability (2)**

(JS: order = [3,0,1,2]; first in that walk that is strictly lower wins; initial pick is Resilience.)

5. Worked example (page demo answers)

6. How the report is produced

1. After Q16, email gate → Show My Results calls go('results') → renderResults().

2. Report is rendered on-page in JS (dial, bars, focus block, four capability cards).

3. Download my result = downloadResult() → window.print() / Save as PDF in the browser.

No server-generated PDF file.

9. Email / PDF decision (default)

1. Users get the nice on-page report + Download my result (print → PDF).

2. GHL email automation = planning only (see ASSESSMENT-FUNNEL-AUTOMATIONS.md).

3. PDF attachment in email = DEFERRED.

4. When Len provides a GHL webhook, forward contact + scores via existing forms waitUntil; still no auto PDF attach unless Leigh insists later.

10. How Leigh changes the logic

1. Edit JS CAPS / TIERS / band() / focus order in the assessment index.html.

2. Update this MD and the Sheet Logic Tree tab to match.

3. Redeploy hub (Cloudflare Pages Direct Upload).

4. Hand Reviewer the live URL for a click-through.

11. Email gate fields (current product)

First name, email, optional role, opt-in checkbox, **cell (Your CELL)**.
Button: Show My Results — shows results and POSTs capture payload to site forms handler (`/`).
